[Mesorah] 2 nusach-hat'fila questions

G'mar chasima tova to all.  I have two questions about nusach hat'fila:

1.  I know someone who, on eating an apple with honey says not "... 
shet'chadesh alenu shana tova um'suka" but instead ".. shet'he hashana 
hazos m'suka k'sapuach bidvash".  Does anyone know of a source for this, 
or has anyone heard of such a nusach?

2.  The Artscroll machzor's yizkor for one's grandmother says "z'kenti": 
zayin-sh'va kuf-segol nun-sh'va tav-d'gusha-chirik-male.  Shouldn't that 
be "z'kenasi" (zayin-sh'va kuf-tzere nun-kamatz-gadol sav-chirik-male)?  I 
mean, you wouldn't say "avedti" (for "my lost object"), would you?
