     The source is the Ran on the Rif in the first perek of K'subos (and it's quoted in the Taz EH 34), that "Rabbeinu Tam z"l higiah," which indicates it was Rabbeinu Tam's change.

Can someone show me a cite which demonstrates that Rabenu Tam deliberately added that word, rather than merely choosing one existing nusach over another?

I'm not necessarily doubting anyone; I'd just like to see it for myself inside.
