The habit of everybody shouting "brichu" was introduced somewhere in 
order to remind the chazen of this, but nowadays it's somehow the 
other way around. BTW, "kudhso" isn't Aramaic for "hakkodoush" (Isr.: 
hakadOsh), but for "hakkoudesh" (Isr.: hakOdesh), so it's even more 
plausibly "... sheim kodshou. Boruch hu...", not "sheim HQBH".

     How, then, does one understand and parse "ana avda d'kudsha 
b'rich hu"?  And what is the function of the dalet in "d'kudsha," if 
it is "hakodesh" and not "hakadosh"?  "Sh'mo shel hakadosh" makes 
sense; "sh'mo shel hakodesh" does not.
