When would you guess that phenomenon started?

The Kitzur in 26:18 already mentions multiple people saying Kaddish.

כשיש הרבה אבלים רחמנא ליצלן, אזי כדי שלא יבא לידי קטטות ומריבות,
נוהגין בהרבה מקומות, שאומרים ב' או ג' ביחד.
