[Avodah] Hashgacha sources

Someone referred me to this list after I asked about hashgacha pratis vs. klalis on another list. I've looked through the thread, but at first glance still haven't found an answer to my questions, so here goes:
 
 1. Does anyone know of any pre-Geonic sources for the terms "hashgacha
 pratis"/"hashgacha klalis"?
 2. Which source would you consider to be the definitive definition for the
 expressions?
